Board Certified Geriatric Case Manager
We employ a Board Certified Geriatric Case Manager who is trained and experienced in care management. Our Geriatric Case Manager is a member of the National Association of Professional Geriatric Care Managers (NAPGCM) and assists older adults or persons with chronic needs or a disability to function at their maximum capacity.
Why use a Geriatric Care Manager?
• Communicate and coordinate with family, doctors and other skilled providers
• Contain costs by coordination of care which ensures unnecessary
hospitalizations, medication management and duplication of services
• Works with complex cases
• Provides short or long term assistance for the needs of individuals that
may be transient or the family is at a distance and wants care coordinated
and communicated
• Works with all resources in the community, which includes, social workers,
doctors, nurses, advocates and other elder care professionals
Every person that we care for is unique.
We work with you (or family members) to
create a specific Care Plan that outlines
your needs, the type of services that will be
provided and any special needs that are
required. Our care and the individuals we
serve range in age from pediatrics to serving
older adults. Care can range from 1-24
hours per day.

Our care providers are experienced
professionals and have hands on training on infection control, nutrition, taking vitals, use of equipment, safe transfers, bathing and grooming, Alzheimer’s, hearing loss and more. We know that one of the greatest long-term needs of older adults and those with chronic illnesses is for in-home custodial care services. We provide In-Home Caregivers that assist with personal care and homemaking.
